I almost bought an old 4 wheel drive ambulance with that in mind but figured the cost to do it right was too high and it was a ford diesel that I am not a fan of

A friend's FIL converted an old city bus. Called it "Wasabus" Took a long time and in the end when he sold it was a very nice, but still unfinished RV. Such a job is a lot more work than it looks like.

You might enjoy reading through some of the build threads over at Expedition Portal forum. There are a handful of ambulance conversions, some even on a 4x4 platform.
