This is a Top Cap EZ Lift. Company is no longer in business, but you can find YouTubes that describe the product. You mount it permanently on the ceiling or rafters of a garage or carport to lift and store a camper shell when you need to remove it from the bed of your pickup, for example to switch from worktime (truck shell) to playtime (4-Wheel Camper). Could also be used to lift a jeep hard top.
Brand new condition, still in the box, never used, have the bill-of-sale. Original price was $299.
Soon after buying it in 2013, we switched to trailer camping, and eventually sold our 4-Wheel. The EZ Lift has been stored in my workshop ever since, dry and out of the elements.
